University of York Student Support & Advice Self-Referral Form
This is a self-referral form for receiving support and advice. The information you provide on this form will be used by Student Life and Wellbeing to determine the best way to address your needs for advice and support. This may include providing you with information by email and/or contacting you to arrange an appointment with a specialist adviser. In some cases we will need to refer you to other internal or external support providers based on your particular circumstances, but we will check with you first prior to doing so. Your data is being processed because it is necessary for the performance of the contract to deliver advice to our students/clients and, where applicable, because you have consented to the processing. We will also create statistical information to monitor the effectiveness of our service and to improve services across the University. Please note that the information you provide to us will not be used to update your central student record. *If you are a parent, guardian, or are enquiring about a student, please note that due to GDPR, we cannot discuss the student's case without their express consent, and will only be able to provide general advice around policies/procedures where consent is not in place. We are also unable to disclose any information that may confirm or deny that a student attends the University. Students can provide their express consent for us to discuss their circumstances with a parent or guardian, either by emailing us at [email protected] or submitting a referral using their University details.
