The plot of IM is mostly the same, but he never reveals that he's iron man, because who is going to believe that when his body is in the hospital. IM2 doesn't happen, palladium poisoning isn't as pressing of an issue without the suit using the arc reactor and Vanko doesn't much care about messing up Tony anymore. So Tony mostly bumps around doing what he wants and protecting people, kind of wondering if the world is better off with Tony Stark in a coma. So come Avengers, everyone thinks Iron Man is some great robotic hero and possibly the last invention by Tony Stark. He gets invited to fight against Loki and meets the Avengers and more importantly Steve. With nothing better to do after all that is done, he ends up hanging out with Steve, who is pretty amazed by the cool "robot". Except then they start falling in love.
